Doctor Notes are needed/help us:

Crutches: Students need MD notes to use crutches or other assistive devices at school.  There

 must be an end date for the crutches or assistive devices.

ER visit:  Doctors notes from ER visits help School Nurses better understand how to accommodate students at school.

Surgery:  If your student is having surgery, please contact your School Nurse to help determine a plan of care for your student.  Notice of two weeks before surgery is necessary to create a plan of care at school in a timely manner.

PE:  If your student needs PE modifications, please contact your School Nurse
